INDIANAPOLIS--(BUSINESS WIRE)-- Elevance Health, along with its affiliated health plans, today announced 45 distinguished medical practices and health systems to be honored in its annual Care Provider Recognition Program. The program spotlights the top one percent of primary care providers in Elevance Health’s nationwide affiliated health plan network of about 7,200 value-based care and evaluated provider partners.

The recognition highlights primary care providers who exhibit excellence in delivering high-quality, coordinated care for members across Elevance Health affiliated employer-sponsored, Medicaid, and Medicare Advantage health plans. Selections are based on key performance measures, including preventive care screenings, immunization rates, chronic disease management, and medication adherence, as well as a strong commitment to continuous learning and clinical excellence.
