Rhea-AI Filing Summary

Longeveron Inc. reported updated ELPIS II Phase 2b results for laromestrocel, tested with Stage 2 palliative surgery in 40 infants with hypoplastic left heart syndrome (HLHS). The trial did not meet its primary endpoint, change from baseline in right ventricular ejection fraction (RVEF) at Month 12. Among 28 infants with baseline and Month 12 cardiovascular magnetic resonance data, mean RVEF decreased 2.6 percentage points in the standard-of-care group and 3.3 percentage points with laromestrocel; the difference was not statistically significant (p=0.68).

A prespecified secondary major adverse cardiac events (MACE) endpoint showed 8 events in the standard-of-care arm and 1 in the laromestrocel arm (P=0.043), a statistically significant result. No treatment-related serious adverse events were identified through 12 months. Longeveron intends to request an FDA meeting to review the results and determine potential next steps for its HLHS program.

How this balance works

Rhea-AI gives every point it takes from this document a weight. Minor counts 1, Moderate 3 and Major 9, so one Major point outweighs several Minor ones. The bar adds up the weights on each side, and when neither side holds more than 65% of the total the balance reads Mixed.

It reads the document as published, with the same rules for every company, and it does not look at what the market expected or at how the stock traded, so a point can be objectively good on a day the stock falls.

Rhea-AI Sentiment measures something else, the tone of the wording.

Filing Explained

FDA's stated position means the missed RVEF endpoint alone cannot demonstrate efficacy for approval; transplant-free survival follow-up remains planned for up to five years.

This 8-K adds a regulatory constraint to the updated ELPIS II results: the FDA had advised that RVEF alone would not be sufficient to demonstrate efficacy for approval, so the missed primary endpoint alone does not settle what evidence is needed for approval.

The trial description says transplant-free survival follow-up is planned for up to five years, leaving that longer-term outcome pending after the reported 12-month assessment.

right ventricular ejection fraction (RVEF) medical
"change from baseline in right ventricular ejection fraction (RVEF)"
major adverse cardiac events (MACE) medical
"pre-specified secondary endpoint major adverse cardiac events (MACE)"
Major adverse cardiac events (MACE) are a grouped measure of serious heart-related outcomes—commonly heart attack, stroke, urgent procedures to restore blood flow, or cardiovascular death—used in clinical trials and safety reports. Investors watch MACE because it summarizes whether a treatment or device causes or prevents life‑threatening heart problems; like a single report card of critical safety signals, it can drive regulatory decisions, market acceptance, legal risk, and a company’s valuation.
intention-to-treat (ITT) population medical
"randomized infants in the intention-to-treat (ITT) population"
The intention-to-treat (ITT) population includes every participant in a clinical trial as originally assigned at the start, analyzed regardless of whether they followed the treatment, dropped out, or switched therapies. For investors, ITT results matter because they preserve the fairness of the original comparison and tend to show how a drug performs in real-world use, so they influence regulatory assessments, market expectations, and a company’s valuation.
intramyocardial dose medical
"single intramyocardial dose of an investigational drug"
transplant-free survival medical
"Long-term follow-up for transplant-free survival is planned"
Transplant-free survival measures how long patients live without needing an organ transplant or dying; it counts the time a treatment keeps a patient functioning without progressing to the point where a transplant is required. Investors watch this metric because it shows whether a therapy can delay or avoid costly, risky transplants — like extending the life of a car with repairs instead of replacing the engine — which can affect a drug’s market value, cost-effectiveness, and regulatory prospects.

Item 8.01. Other Events.

On October 26, 2026, Longeveron Inc. (the “Company”) a clinical-stage biotechnology company developing cellular therapies for life-threatening, rare pediatric and chronic aging-related conditions, issued a press release announcing updated results from ELPIS II, its Phase 2b clinical trial evaluating the Company’s investigational stem cell therapy, laromestrocel, as an adjunct to Stage 2 palliative surgery in 40 infants with hypoplastic left heart syndrome (“HLHS”). The updated results follow planned additional analysis conducted by the Company and the National Institutes of Health (NIH) funded trial investigators.

As previously disclosed, the ELPIS II trial did not meet the primary efficacy endpoint of change from baseline in right ventricular ejection fraction (RVEF) at Month 12. However, based on the secondary endpoints, laromestrocel was associated with fewer adjudicated clinical and safety events than the standard of care arm. Additional review of the pre-specified secondary endpoint major adverse cardiac events (MACE) showed statistical significance with eight events in the standard of care (SOC) arm versus one in the laromestrocel treatment arm (P=0.043).

The Company believes the favorable pre-specified secondary endpoint findings suggest that laromestrocel has the potential to meaningfully improve clinical outcomes of children with HLHS undergoing Stage II surgical palliation. The Company intends to request a meeting with the U.S. Food and Drug Administration (FDA) to review the results to determine potential next steps for the Company’s HLHS development program.

Title: ELPIS II: A Phase 2b Randomized Clinical Trial to Evaluate the Efficacy and Safety of Laromestrocel as Adjunct
Therapy During Stage II Palliation for Hypoplastic Left Heart Syndrome

About ELPIS II

ELPIS II is an investigational Phase 2b, randomized, double-blind, multicenter, two-arm trial conducted by Longeveron in collaboration with the National Heart, Lung, and Blood Institute (NHLBI) through grants from the National Institutes of Health (NIH) (ClinicalTrials.gov: NCT04925024). A total of 40 infants with HLHS were randomized 1:1 to receive a single intramyocardial dose of an investigational drug called laromestrocel that was administered during Stage 2 palliative surgery (bidirectional Glenn or hemi-Fontan procedure) or standard of care surgery alone, and were followed for 12 months. Long-term follow-up for transplant-free survival is planned for up to 5 years.

The trial’s primary endpoint was the difference between groups’ change from baseline in RVEF at 12 months, assessed by cardiac magnetic resonance (CMR). As previously disclosed, FDA has advised Longeveron that RVEF alone would not be sufficient to demonstrate efficacy for regulatory approval.

About Hypoplastic Left Heart Syndrome (HLHS)

HLHS is a rare congenital heart defect that affects approximately 1,000 infants per year in the U.S. Infants with HLHS are born with an underdeveloped left ventricle, which creates a life-threatening condition due to the heart’s inability to pump adequate amounts of blood throughout the body. The current treatment requires infants to undergo a complex three-stage heart reconstruction surgery process over the first five years of their life. Despite staged surgical palliation, long-term mortality and morbidity remain substantial, with progressive right ventricular dysfunction representing a major contributor to adverse outcomes. There is clearly an important unmet medical need to improve right ventricular function in these infants to positively impact both short- and long-term patient outcomes.

About laromestrocel (Lomecel-B®)

Laromestrocel is an investigational living cell product made from specialized cells isolated from the bone marrow of young, healthy adult donors. These specialized cells, known as mesenchymal stem cells (MSCs), are essential to the human endogenous biological repair mechanism. MSCs have the potential to perform a number of complex functions in the body and possibly support tissue repair and regenerative signaling pathways. They also may respond to sites of injury or disease and may secrete bioactive factors that are immunomodulatory and regenerative. We believe laromestrocel MSCs may have multiple potential mechanisms of action that may lead to anti-inflammatory, anti-fibrotic, and/or pro-vascular regenerative responses, and therefore may have broad applicability for a range of rare and aging related diseases.

About Longeveron Inc.

Longeveron is a clinical stage biotechnology company developing regenerative medicines to address unmet medical needs. The Company’s lead investigational product is laromestrocel (Lomecel-B®), an allogeneic mesenchymal stem cell (MSC) therapy product isolated from the bone marrow of young, healthy adult donors. We believe laromestrocel has multiple potential mechanisms of action encompassing pro-vascular, pro-regenerative, anti-inflammatory, and tissue repair and healing effects with broad potential applications across a spectrum of disease areas. Laromestrocel development programs have received five distinct and important FDA designations: for the HLHS program - Orphan Drug designation, Fast Track designation, and Rare Pediatric Disease designation; and, for the AD program - Regenerative Medicine Advanced Therapy (RMAT) designation and Fast Track designation.
