Company Description
Freenome, Inc. Overview
Freenome, Inc. (Nasdaq: FRNM) is an early cancer detection company developing blood-based screening tests designed to find cancer at its earliest and most treatable stages. The company pairs a multiomics laboratory platform with artificial intelligence and machine learning to build screening tests tailored to the biology of individual cancers and to the patient populations each test is meant to reach. Freenome is headquartered in Brisbane, California.
Freenome describes its strategy as Personalized Cancer Detection, combining multiomic blood tests with digital tools that help providers identify eligible patients and deliver guideline-based screening for each individual. By linking screening results to longitudinal real-world data, the approach is built to widen screening participation and refine test performance across successive product generations.
SimpleScreen CRC
Freenome’s first commercial-stage product is SimpleScreen™ CRC, an FDA-approved blood-based screening test for colorectal cancer in adults 45 and older who are at average risk for the disease. The test uses a standard blood draw and detects signals associated with colorectal cancer from cell-free DNA (cfDNA) circulating in the blood. A positive result indicates a signal that may suggest colorectal cancer or advanced precancerous lesions and is followed by colonoscopy. The test is not a replacement for diagnostic colonoscopy or for surveillance colonoscopy in high-risk individuals.
SimpleScreen CRC was evaluated in the PREEMPT CRC study, a prospective clinical validation program run at more than 200 sites that enrolled over 48,000 asymptomatic, average-risk adults scheduled for a screening colonoscopy. With FDA approval the test meets Medicare coverage criteria, which matters for a screening product whose adoption depends on reimbursement.
SimpleScreen Lung
SimpleScreen™ Lung is Freenome’s investigational lung cancer screening test, developed for adults ages 50 to 80 with at least a 20 pack-year smoking history who are not participating in guideline-recommended lung cancer screening. It combines methylation and proteomics technologies with an AI/ML classifier that determines whether a lung cancer signal has been detected. Under the proposed indication a positive result is followed by low-dose CT (LDCT) imaging, and a negative result does not guarantee the absence of lung cancer.
The FDA has granted Breakthrough Device Designation to SimpleScreen Lung. That program is intended to speed the development and review of devices offering more effective diagnosis or treatment of life-threatening or irreversibly debilitating disease, and it gives the company interactive communication and prioritized review during the premarket submission process. Breakthrough Device Designation is not FDA approval, and the test remains investigational.
Technology Platform
Freenome’s platform analyzes circulating cell-free DNA methylation patterns at single-base resolution alongside additional biomarkers, layering proteomics, next-generation sequencing and machine learning classifiers on top. The same base-level methylation and sequencing foundation underlies both the approved colorectal test and the investigational lung program, which is what lets the company extend into new indications without rebuilding the assay from scratch.
Protein and autoantibody capability sits alongside the DNA work. Autoantibodies are immune-system proteins produced in response to the body’s own molecules, and they can serve as early indicators of disease. Freenome brought that capability in through its 2023 acquisition of Oncimmune Ltd, a global immunodiagnostics developer, adding non-tumor-derived signals that widen the view of the tumor microenvironment.
Commercial Partnerships
Commercialization of SimpleScreen CRC in the United States runs through Abbott rather than a Freenome sales organization. The commercial agreement between the two companies gives Abbott exclusive rights to commercialize the test in the U.S., building on the provider relationships, reimbursement expertise and patient support programs behind its existing colorectal screening portfolio, and it carries a $100 million milestone payment to Freenome triggered by FDA approval.
Outside the United States, a License and Option Agreement with Roche gives Roche Sequencing an exclusive option to license Freenome technology for kitted cancer screening assays, including colorectal and lung cancer. The two are developing a decentralized blood test kit covering several cancer indications, meaning qualified local and regional laboratories would run the test near the point of care instead of shipping samples to one central lab, and Roche would sell those kits outside the U.S. if the work succeeds. Freenome is separately evaluating its assays on Roche’s sequencing by expansion (SBX) platform, where availability depends on meeting agreed requirements. Freenome keeps its U.S. rights and ex-U.S. centralized testing, subject to licenses already granted, and Roche holds a preferred-partner right to negotiate should Freenome pursue an ex-U.S. centralized testing partnership.
Pipeline
Beyond colorectal and lung cancer, Freenome is advancing a pipeline of non-invasive single-cancer and multi-cancer screening tests across additional indications, supported by the PROACT LUNG prospective study and further clinical validation work intended to support regulatory submissions. The company’s stated aim is to make cancer screening simpler to complete and to raise participation among the millions of eligible adults who go unscreened.
Public Company Background
Freenome became a public company through a business combination with Perceptive Capital Solutions Corp., a blank-check company sponsored by an affiliate of Perceptive Advisors.
